Perryville, Mo. (KFMO) - The Federal Occupational Safety and Health Administration has issued 10 “serious” citations and fined Gilster-Mary Lee Corporation a total of $91,262 following a worker’s death at the company’s cereal plant in Perry County.
OSHA opened its investigation on June 26, 2025, the same day 38-year-old Nicolas Lopez Gomez of Guatemala died while working at the facility. According to OSHA, the citations stem from multiple workplace safety violations. The most serious citation states that company procedures “did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance.”
The agency has set a deadline of May 15 for Gilster-Mary Lee to correct the cited violations.
